KEY WORKER INFORMATION FORM
If you would like to request a school closure place for your child because you are a critical worker (see Government list of keyworkers), please complete the form below. Your child must not attend school until you have received a confirmation from the school that they may attend. If you are requesting a place for the first time, we may ask your employer to confirm your employment status as a critical worker. We may withdraw the place if your employer does not agree to provide this information.
